Community Navigator Durham area Temporary for 12 months, full time (37.5 hpw), 9 am to 5 pm, Mon to Fri £22,500 per annum with great benefits including Health Cash Plan Home, a place where you belong We have a great opportunity for you to join our Alliance team as our Community Navigator working in and around the Durham area, supporting our customers living with mental health conditions to access and engage with services and organisations, whilst promoting positive mental health and wellbeing. Our team is responsible for representing all the alliance members which include 7 providers, a commissioner and 7 subcontractors. Typical day as a job title Follow up on referrals and connecting customers to relevant services Person-centred planning for each customer Be the link that keeps everyone connected Help change our customer's lives through engagement with their carer’s, and professionals involved in their support network. You bring Great organisational skills, with a calm approach to tasks Be a good communicator using the ‘make every contact count’ approach (Mecc) You’ve worked in a care environment – maybe you’ve cared for a child/parent etc. It’s important that you have a little experience with technology, but we’ll support you with this To get from A to B, you’ll need a vehicle insured for business purposes.
