Lead Support Worker Durham Acute Hospital Discharge and Prevention Service Hybrid role - Community based from the University of North Durham and Darlington Memorial hospitals, our office in Durham and from home. As our Lead Support Worker, you’ll be at the heart of supporting our customers who have complex needs, to develop their skills and live more independently in the community. You’ll make a big difference each day as you help our customers achieve their hopes and aspirations. Amazing we know! Typical day as a Lead Support Worker (known by us as Coordinator) Our service is on offer to any person from Co Durham (excluding Darlington) being discharged from hospital. This could be from UHND, Darlington Memorial or any of the community hospitals in Co Durham. Our service is a mental health service offering support with a variety of support needs. This ranges from housing, homelessness, mental health referrals, support with drug and alcohol misuse, social isolation and finances. Our goal is to prevent bed blocking within hospitals and to prevent readmissions as much as possible. Carrying out risk backssments, support planning, goal setting and regular reviews. Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too. We also have two properties we can use within our service to house customers temporarily while we help them find suitable permanent accommodation. You will help with some housing management tasks for these properties too such as fire checks and making sure they are fully compliant. Fancy going home each day knowing that you have helped.
