Description:

Performs skilled human service work by providing direct person-centered treatment, recovery and/or support services to assigned clients in behavioral health and long-term support programs in the community. These programs may consist of the Comprehensive Community Support Program (CCS), Coordinated Services Team (CST), Community Support Program (CSP) and Children's Long-Term Support Medicaid Waiver Program (CLTS). Is responsible for developing and implementing person-centered service plans, providing service delivery, accessing and transporting individuals to community services, employment programs, and events for assigned clients. The work is performed under the direction of the Coordinated Services Manager and/or Supervisor. This position is about 15-29 hours per week.
