The Social Worker is a member of the Behavioral Health team providing short-term services to patients and families with behavioral and/or psychiatric needs. Hands-on training with opportunities for advancement Position Description Delivery of assessments to determine psychological status, physical functioning and patient vulnerability, including: patient understanding of severity of disease, level of previous emotional functioning, status of current social support, current level of distress/anxiety/depression, current levels of pain, current functioning issues (sleep, pain, food), and current coping strategies. Determine life stressors that could increase cancer distress and impair adjustment including: emotional dysregulation, family dynamics and role changes, financial concerns, insurance and disability issues, spiritual questions, cognitive and behavioral issues. Assist patients with post-treatment adjustment including survivorship care planning, rehabilitation, identifying post-traumatic growth and life-after-cancer psychological concerns. Assist with advance care planning and with end-of-life preparation and care including: resources for advanced directives, considering and identifying palliative care and hospice resource options and identifying legal consultation resources. Required Some experience in the delivery of assessments to determine psychological status, physical functioning and patient vulnerability. Excellent analytical, problem-solving, spoken and written communication and interpersonal skills. Experience with serving clients as part of a multidisciplinary care team Preferred Background in health, hospice or mental health settings delivering evidence-based interventions to clients coping with chronic and acute life-threatening illnesses, with knowledge of medical and psychosocial aspects of cancer and grief, loss and bereavement preferred. Aptitude with assisting patients adjusting to diagnosis including: resource planning (transportation, child care, palliative home care, distance treatment lodging, support groups, etc.), systems navigation and care coordination specific to ovarian cancer. Aptitude with assisting patients accessing social services (Medicaid, Social Security, SSI, financial assistance grants, patient assistance programs etc.) Experience may include patient understanding of severity of disease, level of previous emotional functioning, status of current social support, current level of distress/anxiety/depression, current levels of pain, current functioning issues (sleep, pain, food), and current coping strategies.
