Position Title: Optometrist Address: 1756 Sagamore Rd. Northfield OH Pay rate: $ 131/hour Client: State of OH ODRC - MHA - Northcoast Behavioral Healthcare Openings: 1 Description: Conducts routine, periodic and emergency examinations using specialized diagnostic equipment (e.g., refractometer, indirect and direct ophthalmoscope) to Client defects that can be corrected by prisms, lenses or other optical accessory, determines by examination and evaluation of medical history whether corrective treatment of physician referral is indicated and prescribes, fits, adjusts and repairs glasses for patients. Acts as consultant to other professional & paraprofessional staff regarding forms of eye pathology; evaluates medical records to determine corrective optometric treatment. Plans, schedules and directs comprehensive eye screening programs, prescribes, fits, adjusts and repairs glasses; instructs patients in proper eye care and care of eyeglasses; writes medical and technical reports. Remarks: Duration : At least 3 Months from projected start date Hours/Week 8 Start Time 8:00 AM End Time 5:00 AM Minimum Education Level Doctoral Degree Submissions must include: Resume RTR Healthcare-professional level CPR certification Ohio License If Missing any documents the bid will be rejected If Chosen for position TB test must be uploaded w/in 48 hours of start. Submissions over the max budgeted bill rate will be rejected. 1st Shift, can be between Monday - Friday 8:00 am - 5:00 pm (flexible scheduling) but is simply 1-2 times per month for approximately 8 hours each day.
