A company is looking for a Care Manager - LVN - ICD. Key Responsibilities: Coordinate care for members in collaboration with the Integrated Care Team Review and authorize medical services based on clinical information Manage member medical cases by applying care management principles and practices Required Qualifications: Licensed vocational Nurse (LVN) required 2-4 years of clinical nursing experience 2+ years of experience in care or utilization management 5+ years of experience in healthcare, medical management, change management, and/or leadership Knowledge and/or familiarity with the principles of project and program management
